Jason Reihana has been let out of prison on compassionate grounds, as he has terminal leukaemia.
He murdered his ex-partner Teresa Gunn, and her new boyfriend, in a frenzied stabbing in Tauranga in 2005.
An administrative error means their families were not told of his release - as they were not officially registered as victims.
National leader Simon Bridges was the Crown Prosecutor for the case.
He says there needs to be a serious investigation.
Bridges says it's an appalling breach of victims' rights.
