The man was shot dead by an officer in November last year, after he holed up in a wardrobe with his two young daughters, and held a knife to one's chest.
The four and six year old had nothing to eat or drink for 15 hours.
IPCA chair Judge Colin Doherty says police had no option but to shoot the man, but should have done better for the children.
He says there should've been a way to get food and water to the children, rather than using that to lure the man out.
