    Failures in the care of a woman - whose baby was stillborn


    A scan at Pacific Radiology when the woman was overdue - noted low amniotic fluid and a drop in the estimated fetal weight.

    She was reassured - and told her midwife would get the results.

    After she got help for bleeding and reduced fetal movements, it was discovered a coding error meant the midwife didn't get the results.

    The woman was referred to hospital and the baby was induced and stillborn.

    Deputy Health and Disability Commissioner Rose Wall found the midwife should have followed up and along with Pacific Radiology had breached the Code of Consumers' Rights.

    Both have since made changes.
