The former Wallabies flanker has been unable to shake the injury and will watch Friday night's match against the Sunwolves from the stands.
The 37-year-old is leaving the Reds at the end of the season to take up a short-term contract with English Premiership side Bristol.
Reds coach Brad Thorn says it's unfortunate that Smith doesn't get an opportunity to play for them one final time, but they believe it's in his best interests to listen to his body and sit this one out.
