A new report's found the New Zealand Transport Agency's been asleep at the wheel for some time

An independent inquiry was commissioned last year after concerns about unauthorised vehicle inspectors issuing warrants of fitness.
Three businesses had their licenses suspended for substandard inspections and close to 10-thousand vehicles were recalled for retesting.
Report author Kristy McDonald QC has noted NZTA's failure to prioritise public safety was an example of wider systemic failures that have existed for some time.
Ten of the 25 recommendations in the report have already been fully or partially implemented by NZTA.
