In 2017, an emergency stop button was pushed on the outside of a train, immobilising it.
About 10 taggers in masks then spray painted it.
A report by the Transport Accident Investigation Commission has found Transdev had no procedures in place to deal with such an incident.
Staff have since received training on how to better liaise with emergency services.
