A faulty printer in Hawke's Bay Hospital, was one reason medical equipment missed being sterilised

A review has been released on the February incident, which saw up to 55 patients being treated with unsterilised equipment.
It ha found all three sterilisation checks failed - and staff failed to follow policy.
The errors include, reliance on a broken printer, a sterilising machine switch that was known not to work - and a raft of human mistakes.
Several changes have been made in the aftermath, including replacing the printer and switch.
