It's been ruled a female prisoner died because of inadequate healthcare in a correctional facility

Deputy health and disability commissioner Kevin Allan released the findings today.
The woman had irritable bowel syndrome - and over several months after arriving at the facility, she reported further symptoms.
After she told staff she had black matter in her vomit, it was found in hospital she had advanced gastric cancer.
Allan says he had concerns about the staff's critical thinking - because no one seemed to look at the overall picture, despite the woman reporting health concerns a number of times.
