The coroner's report released today reveals the crash could have been stopped if someone had reported the driver was high on drugs.
Jeremy Thompson and his family were on their way to Whanganui when his vehicle collided with an oncoming car.
The report found he and his partner had repeatedly consumed synthetic cannabis on the day of the crash.
Two Hawera McDonald's drive-thru staff who served the driver - noticed he was "gone" - 30 minutes before the crash, but didn't call Police.
The woman who served them said Thompson was slurring his words and she could smell cannabis in the vehicle.
