South Canterbury DHB has been criticised for its failure to care for a woman in her 60's

Health and Disability Commissioner Anthony Hill has released a report on South Canterbury District Health Board and Pacific Radiology's lack of action.
He's found the woman's appointments were never followed up, which likely delayed her cancer prognosis.
Anthony Hill says when she presented with further symptoms five-years later, there was a five-month waiting time for a semi-urgent scan.
He is recommending the DHB and radiology service review their policies and apologise to the woman's family.
