    A breach of Health and Disability Consumers' Rights in the monitoring of man with cardiac issues


    A 68-year-old who had a heart attack, was given blood thinning medication by staff at a Taranaki public hospital... who didn't know he'd sustained a head injury.

    The man suffered a large brain bleed and died the next morning.

    Commissioner Anthony Hill says the Taranaki DHB lost sight of the man through this process.

    He says he recommends the DHB review its communications to ensure accurate handover between shifts.
