An independent review has been released today, prompted by mail from the alleged Christchurch gunman being published online.
Its 13 recommendations include carrying out regular process audits, and setting up monitoring.
Christine Stevenson says specially trained staff will deal with mail in every prison - by next year.
But she says it would be a bold promise to say harmful mail will never get through.
