The 86-year-old was found with maggots in his wounds -- before dying last March at a Heritage Lifecare rest home in south Auckland.
Investigators found his dressings weren't changed frequently, he wasn't regularly showered and his room was dirty.
Deputy Health and Disability Commissioner, Rose Wall says his care was 'unacceptable' -- and he suffered unnecessarily.
She's ordered a competence review by the Nursing Council of the manager involved.
Heritage Lifecare has accepted the recommendations and outlined the actions it needs to take.
