Deputy Health and Disability Commissioner Rose Wall has released a report - which found a rest home failed to appropriately manage care for a woman whose condition rapidly deteriorated over a period of four days
She was admitted to hospital after she lost consciousness and later died from sepsis and cellulitis.
The report found there was a lack of response to her declining condition, and the rest home failed to contact a doctor earlier as the woman displayed confusion and deterioration.
