The Commission's released its report on the unnamed rest home - managed by Residential Management Limited.
Deputy Commissioner Rose Wall says a resident in their 90s with a catheter and incontinence developed several UTIs - until eventually one killed her.
She says the rest home didn't properly manage the catheter - with staff forgetting to empty the bag.
Wall says Residential Management should apologise to the woman's family - and train staff better.
