A DHB has been reprimanded for failing to identify a woman's "red flag" breast cancer symptoms

The woman, who had a history of breast cancer, turned up with chest pain and shortness of breath, fearing a recurrence - but hospital staff said it was due to her recent breast reconstruction surgery.
After a CT scan was ordered - then cancelled - she had a private scan that showed cancer in her breast and sternum.
Health and Disability Commissioner Anthony Hill has recommended the DHB apologise, train the staff involved - and provide updates on a process to improve communication.
