An investigation into Te Puke company Addiction Food, found a machine used to pack pet food was not properly guarded when a worker's fingers were crushed and burnt in 2018.
WorkSafe's acting chief inspector Danielle Henry says they'd engaged with the company on multiple health and safety issues prior to the incident, and it was highly disappointing.
