A Worksafe investigation's found a triple fatality could have been prevented -- with proper traffic management

Three workers were killed while working on a roadside in the Bay of Plenty in February last year.
It was a tragic day for a maintenance crew cleaning culverts on State Highway 2 on the Matata Straights -- when a truck crashed into their work vehicle -- plowing into three of them.
An investigation found Higgins Contractors Limited hadn't ensured temporary traffic management -- leaving the workers unprotected from traffic.
The company was ordered to pay a 270 thousand dollar fine -- and nearly 500 thousand in reparation.
