The obstetrician-gynaecologist has been found in breach of the Health and Disability code.
During the procedure, the surgeon placed a surgical diathermy pencil on her patient, instead of in a holster - causing three deep burns.
Former Commissioner Anthony Hill says the individual error was not indicative of any system at the private hospital - but it put the patient in harm's way.
He's recommended the Medical Council consider whether to review the surgeon's competence.
