Police accept they should have abandoned their pursuit of a fleeing driver that ended in the death of Christchurch man Kenneth McCaul.
The Independent Police Conduct Authority found officers shouldn't have started the police chase and there were many opportunities when they could've pulled out.
McCaul's husband Owen Fraser says fleeing drivers need to be aware their actions have consequences.
He urges them to think before they act and to stop as soon as police ask them to.
Fraser says he doesn't blame police - and the responsibility lies with the fleeing driver.
