In August 2018, a straddle truck, for lifting containers, tipped over.
It trapped and critically injured 23-year-old Laboom Dyer.
WorkSafe revealed failures in the port's training of drivers, and says some didn't pay attention to the tip alarm.
A bonus system rewarded drivers for moving containers quickly, which Dyer often received despite his high tip-alarm rate.
Dyer had also been using a mobile phone and wasn't wearing a seatbelt.
His young son has been awarded 80 thousand dollars compensation.
