A Health and Disability report has found DHB oversights contributed in the death of a man with pneumonia

The man in his 60s was admitted to hospital with pneumonia and a build-up of fluid around the lungs.
Health and Disability Commissioner, Morag McDowell, says a lack of communication, assessment and monitoring played a part in his deterioration and death.
But she says she can't be sure the outcome would have differed if staff had better followed procedures.
