People linked to border cases in Auckland Orewa had to be re-tested last week, after their initial results went missing.
A medical expert says human error is inevitable in any high volume testing environment when there's manual handling.
Auckland University Professor Des Gorman says it raises concerns for the prospect of a mass-vaccination campaign.
He says four different vaccines, which need different handling, for five-million people could overwhelm a clunky system.
