The Deputy Health and Disability Commissioner has found they breached the Health and Disability Services Consumers' Rights.
The woman, in her seventies, was referred for a CT scan - and the radiologist reported it presented unremarkable findings.
Her symptoms continued and she was diagnosed privately with advanced pancreatic cancer, and died in 2019.
Deputy Commissioner Deborah James says the radiologist failed to provide the woman with reasonable care and skill.
