A probe into Whanganui DHB has found it failed a man who died with an ear infection, after developing a rare but known complication

The man in his 30s visited ED five times over two months in 2019 -- then died from a brain abscess, three days after his final visit.
Deputy Health and Disability Commissioner, Doctor Vanessa Caldwell, says his condition was wholly preventable.
Her report also says the DHB didn't provide a reasonable level of services -- and breached Health and Disability Services Consumers' Rights. She's recommended the DHB and a medical officer write an apology to the man's family.
