An elderly woman with a severe penicillin allergy died after being given antibiotics

The Health and Disability Commissioner's found Northland DHB, a senior medical officer, house officer and nurse breached her rights.
The woman who had a documented allergy was admitted to Whangarei Hospital.
During a ward transfer, the senior medical officer directed the house officer to change her antibiotic.
The woman wasn't informed or asked about allergies, and died.
Recommendations include electronic prescribing, improving handover processes and making it clear prescribers must inform consumers of medication changes and ask about allergies.
