    A finding Waikato DHB failed to give a woman her full X-ray results - who then died with advanced cancer


    She had an X-ray after days of concerns over abnormal lymph nodes, pains and nausea.

    The results were interpreted as an unchanged lung nodule, but a later report showed both lungs were affected and suggested more imaging.

    The patient wasn't told.

    Health and Disability Commissioner, Vanessa Caldwell, says Te Whatu Ora failed in its responsibilities - and could have given the woman more time to manage the illness.
