A coroner found failings by health professionals led to the death of two-year-old Hineihana Mausii in 2013 from leukaemia.
The Hospital sent her home twice.
Southern DHB's Dr Nigel Millar says they accept all the coroner's recommendations.
He says the tragic death has led to better processes for accurately diagnosing children in the ED.
A DHB review found shortcomings included poor supervision of an inexperienced house surgeon.
