He'd been prescribed a liquid hayfever medication - and after two doses had an apparent severe allergic reaction.
A toxicology screen found the presence of an incorrect antipsychotic medicine.
The pharmacy has been found in breach of the Code of Health and Disability Services Consumers' Rights for the dispensing error.
After a MedSafe investigation, the pharmacy made changes including getting new machinery, and increasing staffing.
