    Failures by a medical centre in the case of a man with prostate cancer who later died


    The man's urologist instructed his medical centre to check his PSA levels every six months -- and refer him back if his levels became detectable.

    Over the next three years, five tests showed detectable levels - but the results were not picked up.

    The cancer spread and he died.

    The Aged Care Commissioner has found the centre breached the Code of Health and Disability Services Consumers' Rights.

    It was recommended it make changes, and a GP and nurse write letters of apology.
