    The Health and Disability Commissioner has apologised to Philip Polkinghorne's former mistress for ignoring warnings of his drug use


    Madison Ashton had emailed requesting a confidential call or online meeting to discuss the eye surgeon's substance use and behaviour in June 2021.

    She never received a response.

    At the time, Polkinghorne was on bereavement leave after his wife Pauline Hanna's death in April.

    The Commissioner says due to an administrative error the email was misfiled and didn't reach the Medical Council.
