    Communication and record-keeping failures from two GPs undermined palliative care for a man with terminal lung cancer


    The Deputy Health and Disability Commissioner's highlighted multiple breaches in care for the man in his 80s.

    Records of a telephone consultation with the first GP were missing, and the man wasn't told he was mildly anaemic.

    Iron transfusion wasn't discussed.

    A second GP failed to advise on the use of antibiotics she prescribed during a house call.

    Both GPs have been told to apologise - and the first required to undertake refresher training.
