The Deputy Health and Disability Commissioner's report says Corrections didn't administer the woman prescribed pain medication.
It also failed to prioritise a consultation so a doctor could assess her condition.
The woman ended up in hospital with a viral exacerbation of asthma.
The case has been referred to determine if legal proceedings should be taken.
