A baby died during labour after serious shortcomings in care - from a midwife standing in while the lead maternity carer was on leave

The midwife didn't escalate the woman's care, or appropriately monitor the prolonged first stage of labour.
The baby's shoulder got stuck - which wasn't adequately managed - and post-birth, the midwife didn't promptly identify the woman's sepsis.
Deputy Health and Disability Commissioner Rose Wall has referred the midwife to the Director of Proceedings - to decide if further action's required.
She's also asked the Midwifery Council to consider reviewing her competence.
