A Dunedin woman's suffered an overdose of fentanyl after being given six times her prescribed dosage.
It's also been revealed a man on blood thinners died from a brain bleed after a pharmacist failed to properly manage his medication.
Mangawhai Pharmacy Director Lanny Wong says pharmacists are under a lot of pressure.
She says they're funded for volume over value, so they need to hit prescription targets or they're at a loss for the day.
