    The Deputy Heath and Disability Commissioner's ruled social workers breached the rights of a man, who died by suicide


    The man -- who lived with schizophrenia - was assessed by two psychiatric workers, after an attempt to end his life.

    The assessment missed information like previous suicide attempts and ideation, and relied too heavily on his self-reporting.

    The workers didn't follow safety guidance -- and failed to consult a psychiatrist, or contact the man's wife and support worker.

    He died later the same day.

    Deborah James says the workers' actions represent a serious departure from appropriate care.
