    A care home has been found in breach of the Health and Disability code after the death of an elderly woman with a history of falls


    The woman in her 90s lived at the Chatswood Lifecare Limited home.

    Despite a short-term care plan formed after a first fall, a caregiver briefly left her the same day and she fell again, fracturing her hip and wrist.

    The woman died a few days later.

    Aged Care Commissioner Carolyn Cooper says the organisation didn't adequately communicate the plan, or use reasonable care and skill.

    She's recommending it apologise to the family in writing, and review its processes.
