Health NZ twice wrote to the Midwifery Council -- first after a stillbirth, when it suspended its access agreement with the midwife.
The second time it provided a timeline of events involving six other women.
Two women - including one who developed multiple organ failure - delivered stillborns under her care.
Deputy Health and Disability Commissioner Rose Wall found multiple breaches, and recommended the midwife apologise.
Wall also recommended the midwife do more training if she returns to practice.
