    Concern over the lack of recording of deaths of intellectually disabled people living in care facilities


    An investigation reveals, in the past two and a half years, the Ministry of Health failed to record how 80 per cent of the deaths occurred.

    It follows complaints from the Health and Disability sector that deaths, especially sudden ones, weren't being followed-up.

    Chief Ombudsman Peter Boshier says the Ministry wasn't even aware 30 of the residents had died.

    He says the opportunity for future improvements are lost, if you don't know what could have been repaired.
