Chiefs midfielder Anton Lienert-Brown and Crusaders lock Quinten Strange were forced from the field in separate incidents.
Both appeared surprised to learn they were required to undergo a head injury assessment.
Chief medical officer Eanna Falvey has told Sky's The Breakdown there was an issue in the time taken for the potential concussion event to reach the independent doctor.
Falvey says from 6000 head acceleration events across six matches in the opening round - eight triggered an HIA alert.
