The Mental Health Commissioner has told the Southern DHB it failed to provide a woman with reasonable care

The woman had mental health issues since the 1990s - and her mother reported fears over her daughter's extreme highs and lows.
Her mother confiscated the woman's hunting knives, but she still had to be taken into police custody after harming another person.
Mental Health Commissioner, Kevin Allan, has released a report saying a DHB psychiatrist had earlier assessed the woman as not requiring hospital admission - and recommended she see the community mental health team.
He has found the DHB did not have an adequate cultural care plan and it needs to assess how to integrate its cultural and clinical care.
