NRL head of football Graham Annesley has accepted there were three big blunders during the Warriors' golden point loss to the Penrith Panthers.
Warriors wing David Fusitu'a should not have been awarded a try, with the video bunker missing his hand being in touch.
Secondly, Panthers lock Liam Martin returned from his sin-bin around a minute and a half early due to human error.
Annesley says the third mistake was Penrith five-eighth Jarome Luai being binned for dangerous contact on Roger Tuivasa-Sheck.
