A series of risk factors caused a fatal helicopter crash near the West Coast town of Reefton

The Transport Accident Investigation Commission today published its final report on the circumstances of the crash.
Chief Investigator of Accident, Aaron Holman, says the Commission couldn't conclusively determine what happened in the final second before the accident.
But he notes the helicopter wasn't air worthy, the pilot wasn't licensed to fly in the manner that he was - and the helicopter wasn't maintaining sufficient altitude.
