A New Zealand District Health Board has breached the Health and Disability code, after failing to provide a reasonable standard of care for a suicidal woman

The woman in her 30s had a history of depression, post-traumatic stress, and significant mental health issues.
Her GP made an urgent referral to the DHB's mental service for an assessment, and it was determined she would be reviewed the following morning.
She sadly died before the assessment was carried out.
Mental Health Commissioner Kevin Allan says the DHB had seriously inadequate systems in place at the time of the woman's referral, including no formal process for triaging referrals.
