The incident occurred when two Corrections officers were allegedly assaulted by inmates at Mt Eden remand prison on February 3.
Sources say a single mother and well-regarded officer suffered a traumatic brain injury and was "lucky to survive" the attack.
The prisoner was subject to a "control and restraint" procedure to bring him under control -- when a top-ranked guard allegedly broke the inmate's arm.
A Corrections spokeswoman confirms a staff member is on gardening leave after the incident, but the department won't comment further.
