A 68-year-old woman was waiting to collect whiteware at an Auckland store in August 2022, when the forklift hit her.
Her injuries were so severe, her left leg had to be amputated below the knee.
WorkSafe has found Trade Depot had no effective plan to keep pedestrians and vehicles apart, and the forklift's lights weren't working.
Trade Depot has since complied with two safety improvement notices.
