    A 12 year old ended up needing jaw surgery - after multiple treatment failures by a dentist


    The dentist was not qualified in orthodontic work - but fitted braces then aligners on the girl's teeth over two years, from 2014.

    Three years later, a specialist orthodontist found the child needed braces and jaw advancement surgery - to repair the dentist's work.

    Deputy Health and Disability Commissioner Vanessa Caldwell's found the dentist breached consumer rights.

    The treatment lacked informed consent and clear clinical notes - and the girl should have been immediately referred to the specialist.
